PI exposure was associated with similar increased risk of MI as exposure to cart:
- RR of 1.16/year, about doubling of risk over a 5-year period.
NNRTI exposure was not associated with an increased risk of MI.
Adjustment for exposure to NRTI (as a class) did not affect these results.
These findings suggest that the increased risk - previously reported for cart - is primarily driven by PI exposure.
The effect of PIs on the increased risk of MI is partly - but not completely - explained by the dyslipidemia known to be induced by this drug class.
The lack of increase in risk of MI in more recent years appears to - in part - be explained by the improvement in the extent of dyslipidemia in a targeted population over time.
Limitations:
- relatively fewer years of NNRTI exposure
- insufficient power of the study to examine individual drugs.
