Women & HIV: Black Women in USA; trauma; childhood sex abuse; heart disease in HIV+ women

'Heart Disease - women had a 70% lower risk than men......those with HIV had a 60% higher risk of cardiovascular disease than people without HIV or other severe chronic diseases.....Among people in their 70s, a significantly higher proportion of those with HIV (over 80%) than controls (about 40%) met the high-risk definition'.....

CROI: HIV Incidence in Women at Risk for HIV Acquisition in the United States: HPTN 064 (ISIS Study): 5x higher HIV rates in USA women of color than CDC estimates, comparable to HIV rates in sub-Saharan Africa. Webinar Update see details below. - (03/14/11) study in 'Hot Spots' - Atalanta, NYC, Baltimore, Newark, Wash DC, No Carolina

CROI: Black Women in US Have Doubled Risk of AIDS Death, Regardless of Adherence - written by Mark Mascolini - (03/7/11)

- Trauma Drives HIV Epidemic in Women - (03/20/12)

"The findings demonstrate highly disproportionate rates of trauma exposure and recent PTSD in HIV-positive women compared to the general population of women. For example, the estimated rate of recent PTSD among HIV-positive women is 30.0% (95% CI 18.8-42.7%), which is over five-times the rate of recent PTSD reported in a national sample of women. The estimated rate of intimate partner violence is 55.3% (95% CI 36.1-73.8%), which is more than twice the national rate. Studies of trauma-prevention and trauma-recovery interventions in this population are greatly needed"

- Recent Trauma is Associated with Antiretroviral Failure and HIV Transmission Risk Behavior Among HIV-Positive Women and Female-Identified Transgenders - (03/20/12)

- Study of the Day: 1 in 4 Adults With HIV Were Sexually Abused as Kids - (03/23/12)

- The Pervasive Effects of Childhood Sexual Abuse: Challenges for Improving HIV Prevention and Treatment Interventions EDITOIRAL - (03/23/12) childhood trauma not only confers increased vulnerability for HIV infection among sexual minorities (64% of the men in the sample identified as MSM) but also may interfere with optimal disease management/self-care behaviors among those already infected

- Childhood Trauma and Health Outcomes in HIV-Infected Patients: An Exploration of Causal Pathways - (03/23/12)

"greater lifetime trauma exposure was associated with all outcomes considered (Table 3). Each additional type of lifetime trauma was associated with 18% increased odds of unprotected sex, 13% (3% to 24%) increased odds of ARV nonadherence, 12% (4% to 21%) increased odds of hospitalization, 14% (6% to 22%) increased odds of ED use, 13% (5% to 22%) increased odds of more than 4 days in bed, a 12% (0% to 24%) increased hazard of HIV disease progression, and a 0.8-unit (0.4-1.2) lower SF36 physical health score."

Associations of Sexual Identity or Same-Sex Behaviors With History of Childhood Sexual Abuse and HIV/STI Risk in the United States - (03/23/12) "Men and women sometimes or frequently abused had significant increases in risk for HIV/STI incidence compared with those not abused"

This study was reported at the 3rd Lipodystrophy Workshop in Athens (Oct 23-26, 2001). I reported in an earlier report from this conference that Vincent Mooser

Hormonal Contraceptives Tied to Non-AIDS Disease or Death in HIV+ US Women: 'doubles Non-AIDS Risks' (01/11/12)

Of non-AIDS diseases analyzed, cardiovascular disease (hypertension in 90%, myocardial infarction in the rest) was significantly more frequent in contraceptive users than in nonusers (12% versus 5%). .......The retrospective Nashville study involved 467 hormonal contraceptive-eligible women.....hormonal contraceptive use doubled the risk of a new non-AIDS disease (hazard ratio [HR] 1.9, P = 0.02) and nearly doubled the risk of a non-AIDS disease or death (HR 1.7.....Focusing only on injectable contraceptive users, the researchers again found higher risks of a new non-AIDS disease (HR 2.0, P = 0.03) and a new non-AIDS disease or death (HR 1.9, P = 0.03). Oral contraceptive use by itself did not independently affect the risk of AIDS, a non-AIDS disease, or death.....Although hormonal contraceptive users were generally healthier at baseline, the researchers concluded, they had higher risks of non-AIDS disease and non-AIDS disease or death. The researchers cautioned that their findings are limited by the retrospective nature of the study, by lack of hormonal contraceptive adherence data, and by not including osteoporosis or body mass index in the analysis.
