Early Viral Rebound After T20 Discontinuation in HIV-infected Patients on Long Lasting Supression

BACKGROUND: The purpose of this study was to assess whether T20 can be stopped after a long and sustained viral suppression. In addition, we anakyzed different factors associated with the maintenance of viral of viral suppression after T20 interruption.
METHODS: HIV-infected patients with at least 9 months on viral suppression under T20-containing approaches were recruited. We collected time with suppressed viral load and on T20 treatment as well as CD4T, CD8+/CD38+ cells, HIV RNA viral load and active drugs according to Genotype Sensitivity Score (Bayer, guidelines 8.0) at baseline of T20 therapy.
RESULTS:
17 patients were randomized to stop T20 (7 pts, group 1) or to continue treatment (10 pts, group 2). G1 maintain viral suppression for a median (IQR) of 18 months (14;28) versus 10 months (9.4;447) versus G2. Groups were well balanced wuth respect to nadir CD4 cekks [G1: 62 (5;118), G2: 95 (26.75'157.5)], baseline CD4 cells [G1:159 (73.4;447) versus g2: 176.5 (96.6;302)] and viral load when started T20 (median, log10, IQR)[G1: 4.34 (4.17;4.88) G2: 4.63 (4.04;5.22)]. Number of active drugs at baseline of T20 containing approaches were (median, IQR): G1: 0.5(0,2); G2: 1 (0.75;2.25). At 12 week followup, viral rebounhd occurred in 3 patients from G1 and 0 in G2, Regarding differences between responder(maintain viral suppression) and non-responder patients (viral rebound) from G1, both showed a very similar percentage of CD8+/CD38+ cells at baseline (76% vs 71%). Active drugs at baseline of T20 strategies were 0,5 for nonresponder and 0 for responder patients, respectively.
CONCLUSIONS: Despite the sustained viral suppression in our patients, discontinuatioin of T20 shows a risk of early viral rebound. No differences between CD8+/CD38+ cells nor active drugs at baseline were detected in our patients regarding the maintenance of viral suppression at 12 weeks after T20 discontinuation.
