from Jules: Although the USA DHHS ART Treatment Guidelines recommend ART for all HIV-infected to reduce risk of disease progression with a slightly lower grade of recommendation of BIII for >500 compared to AII for 350-500, at IAS WHO updated their ART guidelines with a recommendation to treat when CD4s fall below 500 as previously while the previous WHO recommendation, set in 2010, was to offer treatment at 350 CD4 cells/mm3 or less.
