PLWH & Fatty Liver Disease, NAFLD. Lifestyle Interventions

In HIV fatty liver disease may be different. Fr sure, many PLWH are obese or overweight & thus weight loss, diet & exercise are important & lifestyle interventions. But we do know that even in the field of mono- fatty liver lean NAFLD does occur. The mechanisms fr causation of fatty liver in HIV may be different & may include HIV or ART related factors & lean NAFLD may be important to consider. Nonetheless the concern about ftty liver & NASH gets little attention in HIV bth among PLWH & among doctors, researchers & clinicians. Little screening in the HIV clinic occurs and little referrals to liver specialists for this issue occurs. With the HIV aging population increasing very quickly fatty liver is a concerning comorbidity deserving attention & consideration. The CDC reported that in 2019 200,000 PLWH in the USA were over 60 years old & that is expected to double to 400,000 by 2030, no doubt fatty liver will be a problem for some PLWH. Jules Levin
